What this site collects, and what we do with it.
This describes what this website actually does. It is short because the site does little: there is one form, one advertising tag, one analytics tag, and no accounts and no payments. Everything that runs is named below.
Please do not send patient information. Nothing on this site is a place for protected health information. The message box on our contact form is for describing your program and your question — not individual patients, prescriptions or claims.
If you need to discuss specifics that involve patient data, tell us and we will set up an appropriate channel under a business associate agreement first.
When you fill in the contact form
The form at /start asks for your first name, last name, facility and email address, and gives you an optional box for your question. All of it goes to us so that a person can reply.
Alongside those fields, the request carries two things we do not ask you to type: the IP address the submission came from and the country our host derives from it. We keep them because they are the only defense against a form being used to send bulk spam through us.
Submissions arrive as a notification in our internal systems and, where email delivery is enabled, as an email to us. They are not written to any database and they are not added to any advertising audience.
What happens automatically on every page
- Server logs. Our host records requests, including IP address, in the ordinary course of serving the site.
- Fonts. Typefaces load from Google Fonts, so Google receives your IP address when a page loads. We do not send Google anything else.
- Your light or dark choice. Stored in your browser under
prox-theme. It never leaves your device and it is not an identifier.
Advertising, and the LinkedIn tag
We advertise on LinkedIn. To see whether those ads bring anyone here, the LinkedIn Insight Tag runs on our homepage and on the contact page. It sets cookies and reports your visit to LinkedIn, which uses it to measure our campaigns and may use it to show you ads elsewhere. That makes LinkedIn an independent recipient of that data, not just a processor acting for us.
You can turn it off at the source in your LinkedIn advertising settings, and a tracker blocker or a browser that blocks third-party cookies will also stop it.
Analytics, and what we asked Google not to do
We use Google Analytics 4 to see how people arrive and which pages they read. It sets cookies and sends Google your IP address, the pages you visited and basic device information.
It runs on every page of this site. Because we want to understand a visit that starts on a phone and finishes on a laptop, we have Google Signals enabled, which means Google may associate your visit with your Google account and may use it to personalize ads you see elsewhere. We would rather say that plainly than describe this as measurement-only when it is not.
You can opt out entirely with Google's browser add-on, and any tracker blocker will stop it too.
The exposure calculator
The calculator runs entirely in your browser. The purchase figure you enter, and every assumption you change, stay on your machine — they are never transmitted to us and we never see them. That remains true with analytics running: it records that the page was opened, never the figures typed into it. The page carries no advertising tag.
Who else handles it
We keep this list short on purpose, and it is the whole list:
- Vercel — hosts the site and runs the form handler.
- Slack — where a form submission arrives as an internal notification.
- Resend — sends the notification email, where email delivery is enabled.
- Google Fonts — serves the typefaces.
- Google Analytics — the analytics tag described above.
- LinkedIn — the advertising tag described above.
What we do not do
We do not sell your information. We do not license or share it with drug manufacturers, pharmacy benefit managers, or their agents. There is no second customer for it. Nothing you send through this site is used to train a model.
How long we keep it
We keep contact details and your message for 24 months after our last exchange about your program, then delete them. Ask us to delete them sooner and we will. Server logs and LinkedIn's own data follow those providers' retention schedules, which we do not control.
Your choices
Write to us at s.johnsen@proximityhealth.com and we will tell you what we hold about you, correct it, or delete it. Depending on where you live you may also have a statutory right to those things, and to object to advertising profiling; we will honor the request either way rather than asking which law you are relying on.
Changes
If we add a tool that collects something new, this page changes in the same release. The date below is the date of the version you are reading.
